If by US Mail please send a written request to James Cerebral Palsy Foundation 9562 Winter Gardens Blvd STE D # 328 Lakeside CA 92040. Please include your full name, mailing address and amount of your donation,. and once we confirm the donation is legitimate we will mail you a physical receipt to the address you provided. PLEASE NOTE: You can expect a physical receipt within 7 to 10 business days.
